What do I enter for ‘Surname’ and ‘Given Name’?

Your surname is your last name and your given name is your first and middle name (if any). Make sure the name you give is identical to the name on your current passport.

When filling out papers, especially those linked to travel and immigration, "surname" and "given name" refer to your family name (last name) and first name(s).
